From the Milan international airport (Milano Malpensa) you have two options to reach Locarno:
By train via Gallarate-Luino-Cadenazzo
The train (with final destination Bellinzona) leaves from Malpensa Terminal 1 at 5.50, 7.50 and every two hours from 11.50 to 19.50.
You must change in Cadenazzo (the last stop before the terminal station of Bellinzona) and take the connecting train to Locarno.
Total travel time is slightly under 2.5 hours.

By shuttle bus to Bellinzona and train to Locarno
You must first reach Bellinzona by shuttle bus (Malpensa Express). The shuttle runs every 2 hours, the trip takes about 2 hours and costs CHF 65.- for a round trip. Reservation (from the website) recommended.
The shuttle leaves from Terminal 1, door 4, parking 21 and from Terminal 2, parking 14.
From Bellinzona you can reach Locarno by train. There are connections every 20-30 minutes and the trip takes approximately 20-25 minutes.
Booking or advance purchase of tickets are possible but not obligatory.
